Abstract

الهدف: دراسة انتشار الأذيات الزرقية لدى مرضى التوسف الكاذب , ودراسة قيم ضغط العين لديهم وعلاقتها مع هذه الأذيات. الطرائق: تمت دراسة 75 عيناً مصابة بالتوسف الكاذب ومقارنتها مع 75 عيناً طبيعية لمرضى مقاربين في العمر, كما شملت دراسة 21 مريضاً لديهم توسف سريري بعين واحدة وتمت مقارنة العينين ,وشملت المقارنة دراسة العصب البصري بجهاز HRT)) Heidelberg Retina Tomograph وقياس ضغط العين الوسطي ومقدار تأرجحه. النتائج: قياسات العصب البصري التي تمت مقارنتها شملت: مساحة العصب, مساحة الحافة العصبية الشبكية, مساحة القرص, سماكة طبقة الألياف العصبية حول الحليمة, نسبة التقعر العمودي إلى القرص, نتيجة تحليل Moorfields Regression Analysis (MRA), ونتيجة تحليل(GPS) Glaucoma Probability Score. وكان الفرق هاماً إحصائياً في جميع هذه النتائج بين المرضى والشواهد,كما كان الفرق هاماً في وسطي ضغط العين ومقدار تأرجحه.أما لدى المرضى المصابين بعين واحدة فكانت قياسات العصب البصري متقاربة دون فروق هامة, ولكن الفرق كان هاماً في وسطي وتأرجح ضغط العين. الخلاصة: العيون المصابة بالتوسف الكاذب ذات خطورة عالية لحدوث أذية زرقية في العصب البصري ويزداد احتمال هذه الأذية بزيادة ضغط العين وتأرجحه, كما تشمل الخطورة العين الأخرى السليمة في الإصابة أحادية الجانب, ما يؤكد أن التوسف الكاذب مرض جهازي ذو تظاهرات عينية. Aim: to study the prevalence of glaucomatous injury in pseudoexfoliation patients, and the relation of these injuries with IOP values. Methods: 75 PXS eyes were compared with 75 age matched control eyes, and so 21 unilateral PXS patients were studied by comparing their two eyes. The study contained ONH study with Heidelberg Retina Tomograph (HRT), and mean and range of IOP values. Results: ONH parameters which were compared between patients and controls included: disk area, rim area, cup area, mean RNFL thickness, vertical cup\disk ratio, MRA result, and GPS result. The difference was significant in all these parameters between patients and controls, also it was significant in mean IOP and its fluctuation. For unilateral PXS patients, no statistically significant difference was found in ONH parameters between the two eyes, but there was in IOP mean and range. Conclusion: PXS eyes are at high risk for developing glaucomatous optic nerve injuries, which were more probable with raised IOP mean and range; the risk also involved the other normal eye in unilateral PXS, certaining the systemating nature of pseudo exfoliation disease.
