تغير مؤشر المقاومة للشرايين داخل الكلية و تبدلات الرشق الحالبي عند مرضى القولنج الكلوي قبل وبعد التدبير
Abstract
أجريت الدراسة في مشفى الأسد الجامعي في اللاذقية بين عامي/2006-2007/ شملت الدراسة/70/ مريضاً راجعوا المشفى بقصة قولنج كلوي حاد,و تم إجراء الاستقصاء الشعاعي لجميع المرضى عن طريق عدة فحوص:
1- التصوير بالأمواج فوق الصوتية مع الدوبلر و الدوبلر الملون مع دراسة الرشق الحالبي و حساب قيمة مشعر المقاومة /RI/للشرايين القوسية داخل الكلى المصابة.
2- صورة شعاعية بسيطة للجهاز البولي من دون تحضير.
3- إجراء تصوير ظليل للجهاز البولي/IVP/ لبعض المرضى الذين تعذر مشاهدة سبب الانسداد الحالبي لديهم باستخدام التصوير بالأمواج فوق الصوتية والصورة البسيطة.
أظهرت الدراسة أن قيمة مشعر المقاومة للشرايين داخل الكلى المصابة(RI) كانت مرتفعة في/16/حالة مقابل /54/حالة مع قيمة(RI)> /0.70/.
كما أظهر الدوبلر والدوبلر الملون قدرة على كشف و دراسة حالات الانسداد الحالبي الحاد في/16/ حالة فقط ,وهي الحالات التي ترافقت مع انسداد حالبي تام,بينما فشل في كشف حالات الانسداد الحالبي الجزئي, وهي في دراستنا /54/ حالة.
للدوبلر قدرة على كشف حالات الانسداد الحالبي التام,بينما يعجز عن حالات الانسداد الحالبي الجزئي.
This study took place at Al-Assad University Hospital between 2006-2007 in Lattakia. The study included 70 patients with renal colic. Patients did undergo diagnostic radiological procedures:
1-Urinary ultrasonography with use pulse and color Doppler with study of ureteric jet and resistance index.
2-K U B.
3- I V P.
The study shows that rate incidence with males higher than that with females. The most affected age group in our study was that between (20-40) years. Ureteric jet was negative in/16/ cases and /16/cases of complete ureteric obstruction. 66 cases showed dilatation of collecting system while in 4 cases collecting system were normal.16 cases had an increase in affected intrarenal arteries' resistance index, while there were 54 cases with RI less than 0.70. Doppler and color Doppler showed a value in detecting and studying cases of acute renal obstruction in 16 cases only, which have complete ureteric obstruction. Doppler and color Doppler failed in detecting cases of partial ureteric obstruction 54 cases in our study. Doppler can detected cases of complete renal obstruction, while fail in partial renal obstruction.
