معطيات الإيكو في التهاب البربخ
Abstract
أجريت الدراسة في مشفی الأسد الجامعي في اللاذقية بين عامي 2008-2007 شملت الدراسة/94/ مريضاً كان لديهم اشتباه سريري بالتهاب البربخ وكان الهدف من الدراسة إبراز دور الايكو العادي والدوبلر الملون والنبضي في تشخيص إلتهاب البربخ كونه وسيلة آمنة وغير راضة وسريعة, وسهلة الإجراء وذات تكلفة اقتصادية مقبولة .وقد تم تاكيد التهاب البربخ بالإيكوعند 62 مريضا فقط ،وقد اعتمدنا في تشخيص التهاب البربخ المعطيات الصدوية الآتية :زيادة حجم البربخ(لوحظت عند 91,94% من المرضى)، وعدم تجانس البنية الصدوية (79,03%)،وتسمك جدار الصفن(59,68%)، ووجود قيلة مائية مرافقة(24,19%)،وفرط توعية البربخ(%95,16)،ونقص مشعر المقاومة (Resistive Index ) في الشريان الخصوي لاقل من 0,5 (96,77%).إن نسبة الإصابة أكثر ماتكون في سن35و60عاما(50%)،وعند المتزوجين(67,74%).بينت الدراسة أن للتصوير بالإيكو العادي والايكو الدوبلر الملون والنبضي واستخدام مشعر المقاومة (Resistive Index ) دوراً اساسياً وهاماً في تشخيص التهاب البربخ بشكل باكر نسبياً وتفريقه عن الآفات الأخرى التي تصيب الصفن وخصوصا انفتال الخصية.
The study was conducted on 94 patients expected to have epididymitis at Al-Assad Hospital in Latakia (2007-2008). The aim was to highlight the role of ultrasound in the diagnosis of epididymitis as a safe, easy, fast, and cheap means of diagnosis. Epididymitis was confirmed by sonography in 62 patients based on the following criteria: epididymal enlargement (91.94%), heterogeneous echogenicity of epididymitis (79.03%), scrotal wall thickening (59.68%), associated hydrocele (24.19%), epididymal hyperemia (95.6%), and the resistive index was less than 0.50 (96.77%). It is noticed that epididymitis occurs mostly in 35-60-year old men (50%) and among the married people (67.74%). The study showed that Gray-scale, Color, and Pulsed Doppler sonography (using RI) have an essential and important role in the early diagnosis of epididymitis and its differentiation from the other scrotal lesions especially testicular torsion.
