قياس الضغط الشرياني المستمر في تشخيص و تدبير ارتفاع التوتر الشرياني دراسة مقارنة على عينة من مرضى الممارسة الطبية في مدينة اللاذقية
Abstract
تقوم الدراسة على إجراء مقارنة مستقبلية Prospective بين قياس الضغط الشرياني في العيادة وقياس الضغط الشرياني المستمر لـ 24 ساعة عند عينة من مرضى ارتفاع التوتر الشرياني الخفيف والمتوسط الشدة والمكتشف حديثاً في العيادات الخارجية الخاصة و مشفى الأسد الجامعي في اللاذقية بين عامي 2006 و 2009. وذلك لأظهار إلى أي مدى يمكن لقياس الضغط الشرياني المستمر أن يكون بديلاً أكثر دقة عن قياس الضغط الشرياني في العيادة في تشخيص و تقييم ارتفاع التوتر الشرياني القريب من الطبيعي
شملت الدراسة 164 مشارك ( 89 ذكور و 75 إناث ) متوسط أعمارهم 38 سنة ( 17 – 54 سنة )
و بناءً على المقارنة فإننا وجدنا :
إن 20.7 % ( 34 شخص ) من المشاركين كان لديهم ارتفاع توتر شرياني بتأثير الرداء الأبيض
( WCH ). وأن نسبة حدوث ارتفاع التوتر الشرياني المقنع ( M.H ) كانت 13.4 % ( 22 شخص ). و ارتفاع التوتر الشرياني المؤكد كانت 18.3 % ( 30 مشارك ). أما بقية المشاركين 47.6 % ( 78 مشارك ) فكان تشخيصهم طبيعي الضغط الشرياني في العيادة و بالقياس المستمر .ABPM
بعد قياس الضغط الشرياني المستمر فإن التشخيص للضغط الشرياني قد تغير عند 40 % من المشاركين
والمعالجات بخافضات الضغط قد تبدلت عند ( 83 % ) ..
This study involved a prospective comparison between office blood pressure and ambulatory blood pressure monitoring in consecutive patients (n=164) with newly diagnosed mild to moderate hypertension; they were selected from outpatient surgeries in Lattakia, Syria. We therefore conducted a prospective study to determine the impact of routine ambulatory BP monitoring on the diagnosis and management of hypertension in our population and the value of other basic characteristics in predicting ambulatory BP results. 20.7 % of the participants had "white coat" hypertension (high clinic BP readings, but not elevated by ABPM). The prevalence of "sustained" hypertension (i.e., high readings in the clinic and by ambulatory BP monitoring) was 18.3%. 13.4 had masked hypertension(normal clinic BP readings, but elevated by ABPM). As for the rest of the participants (47.6 %), their diagnosis was normotensive. Data from the ABPM study affected the diagnosis and management of participants; the diagnosis changed in 40%, and the management in 84%.
