دراسة سريرية لاَفات الطحال الجراحية

Abstract

أجريت الدراسة في مشفى الأسد على /65/ حالة بين عامي تشرين الأول 2000 وتشرين الأول 2009م.

- بالنسبة لعمر المرضى: فقد كان أصغرهم بعمر السنتين بينما كان أكبرهم بعمر 62 سنة.

- بالنسبة للجنس: في ضخامات الطحال الجراحية كانت نسبة الذكور 61.7% ونسبة الإناث 38.2% وبلغت النسبة 1.6/1.

- إن أسباب الضخامة الطحالية: تلاسيميا /10/ حالات, فرط تصنع بدئي بالطحال /3/ حالات, داء تكور حمر وراثي /2/ حالة, تليف طحال /2/ حالة, فقر دم منجلي /1/ حالة, ابيضاض نقوي مزمن /1/ حالة, ابيضاض دم لمفي مزمن /1/ حالة, ابيضاض دم بالخلايا المشعرة /1/ حالة, داء هودجيكن /1/ حالة, اللشمانيا /1/ حالة, كيسات طحال /1/حالة, لمفوما غير هودجكين /1/ حالة, احتشاء طحال /1/ حالة.

- في رضوح الطحال: نسبة إصابة الذكور 77.4% مقابل 22.5% للإناث.

- عن درجة الضخامة الطحالية: ضخامة من الدرجة الخامسة بنسبة 52.9% ،ضخامة من الدرجة الرابعة بنسبة 35.2%.

- تمزق وتهشم الطحال هو المسيطر بنسبة 74.1% ثم جروح الطحال بنسبة 12.9%، أما إصابة النقير فكانت نسبتها 9.6% وأخيراً الكدمة بنسبة 3.2%.

- وفي طرائق العلاج كان استئصال الطحال هو السائد وبلغت نسبته 90.3%، أما خياطة الطحال فبلغت نسبته 6.4%.

والمراقبة بعد إجراء العمل الجراحي مع إبقاء الطحال فبلغت نسبتها 3.2%.

There are 65 cases in the University Al-Assad Hospital, Department of Surgery, during the period 2000—2009, which have the two surgical splenomegaly and splenic trauma.

The causes of surgical splenomegaly numerous. In our study, we found that:

  • · Thalassemia: 10 cases.
  • · Initial hyperplasia: 3 cases.
  • · Hereditary spherocytosis: 2 cases
  • · Falciform anemia: 1 case
  • · Splenic fibrosis: 2 case
  • · Chronic myeloid leukemia: 1 case.
  • · Chronic lymphoid leukemia: 1 case. Hudgken disease : 1 case
  • · Leshmania : 1 case.
  • · Splenic cysts: 1 case.
  • · Non hudgken lemphoma : I case.

Patients age the oldest one 62 years, the smallest 2 years. In splenic trauma degrees of splenic injuries:

  • · Splenic fragmentation 74.1 %
  • · Splenic wounds 12.9 %
  • · Hilar injuries 9.6 %
  • · Splenic bruis 3.2 %

The rate of splenecctomy is 90.3 %, while suture of spleen 6.4%, observation 3.2 %.
