دور دوبلر الشريان السري في تشخيص نقص النمو الجنيني الناتج عن سبب وعائي
Abstract
يعتبر نقص النمو الجنيني داخل الرحم مشكلة سريرية هامة نظراً لكثرة حدوثه وكثرة الاختلاطات التي ترافقه أثناء وبعد الولادة ومن ضمن الأسباب العديدة لنقص النمو الجنيني داخل الرحم السبب الوعائي الذي يمكن كشفه بإجراء دوبلر الشريان السري ، تنتج التغيرات المرضية بالدوبلر عن ازدياد مقاومة السرير الوعائي المشيمي التي يمكن التعبير عنها بازدياد مشعر مقاومة الشريان السري .في مخطط الدوبلر الموجة الانبساطية قد تنخفض أو تنعدم أو تنقلب ، وتمثل الحالتان الأخيرتان الحمول ذات الخطورة العالية والإنذار السيء . يهدف كشف و مراقبة هذه الحالات بالدوبلر لإنقاص الوفيات والإمراضية أثناء وبعد الولادة .
Intra-uterine growth retardation is considered as a real clinical problem because of its high incidence and prenatal and postnatal complications. Of the numerous causes of IUGR is the vascular cause which can be detected by the umbilical artery Doppler. The pathological changes in umbilical artery Doppler are caused by the increased resistance of the placental vascular bed which is expressed by the increased resistance index RI. In Doppler waveform, the diastolic wave may be decreased, absent, or reversed, The last two cases are of high risk and bad prognosis. Doppler diagnoses and monitoring of these high risk pregnancies aim to decrease prenatal and postnatal fatalities.
